I confirm that I understand the conditions of use of the card:

• I confirm that I am employed by a restaurant/retailer within The Lexicon and I will use my card responsibly
• The Lexicon can contact my manager to confirm the status of my Employment at The Lexicon
• Misuse of the card will result in withdrawal from the scheme
• The discounts are given at the managers discretion and the Stores Manager’s decision is final
• This Card is not a cheque guarantee or a credit card and there is no cash equivalent
• This card remains the property of The Lexicon, who reserve the right to decline issue, withdraw or update/amend the card at any time
• I am happy for The Lexicon to use my personal data to contact me with new and updated discounts and offers at The Lexicon.
• My personal data will be stored securely by The Lexicon, for the length of membership to The Lexicon Perks scheme and they may contact me when my card is due renewal
• You can withdraw your consent at any time by submitting your request to info@thelexiconbracknell.com and returning your Lexicon Perks Card to The Lexicon, The Lexicon Management Suite, The Avenue Car Park, Bracknell, RG12 1AP
• Replacement cards will incur a £5 donation to The Lexicon Charity Fund. Payable in cash at The Lexicon Management Suite.
