In order for The Lexicon, Bracknell and Bracknell Forest Council to comply with data protection and confidentiality regulations, it is necessary for us to obtain written consent that we can use photographs that may feature you on our website, social channels and media.

You should only agree to us using your photograph, film and words if you feel completely happy with us doing so and you are under no pressure or obligation to sign this consent form.

How We May Use Your Photograph, Film and Words

We may use your photograph, film and words to publicise events, in newspaper articles, magazines and other media such as websites, social media, information leaflets, electronic newsletters and presentations.

You can contact The Lexicon, Bracknell at any point and ask us not to use your photograph, film or words. We will ensure that you are aware when we take your data for publication how we will use the data and how long we will use this information. Remember that once an article is published and in circulation (especially online) it may be copied and used by others. If you ask us not to use the photograph, film and words in future we will comply with your request and will do our best to stop others doing so, but we cannot guarantee this.

In the case of people who cannot give their consent (such as children and vulnerable adults), a representative such as a parent, guardian or carer, who represents the person in his or her best interests and demonstrates approved authorisation, must sign this form.
